Your practitioner signs the order form, and you send it to us here. Your file goes straight into Sleep Care's secure, HIPAA-compliant storage.

It is printed on your order form and shown on the order page (it starts with SC-).

Only needed if you do not have a reference, so we can match your order.

The order must include your full name, date of birth, your practitioner's signature and the reference. Need the form? Download the order form.

Prefer fax? Your practitioner can fax the signed order to 866-291-8990. Please do not send orders by regular email.
