Home sleep apnea study patient agreement
I. Consent and release
I give my permission to Sleep Care, Inc. staff to complete the home sleep apnea test. I hereby expressly waive any and all claims which I might, at any time, have against Sleep Care, Inc., its employees, relating to this procedure. I understand the risks associated with sleep apnea and do not hold Sleep Care, Inc. responsible if I decide to refuse treatment.
II. Financial responsibility
I am personally responsible for the payment of all charges incurred through Sleep Care, Inc. I understand Sleep Care, Inc. will not be billing my insurance.
III. No refunds after shipment
I understand that once the home sleep test device has been shipped to me, my payment is not refundable. This applies even if I do not use or wear the device, use it incorrectly, change my mind, or if the device is lost or damaged after it is delivered to me. I understand I am responsible for the device once it is delivered, and that Sleep Care, Inc. is not required to replace a device that is lost, damaged or not used.
IV. Use within 60 days
I understand that the home sleep test device is disposable and is not returned. I must wear it for up to 3 nights within 60 days of the date it is shipped to me. If I do not, Sleep Care, Inc. is not obligated to process and interpret my study.
V. Authorization for release of information
I authorize Sleep Care, Inc. to release medical information to any practitioner treating me related to this test or for related follow-up.
VI. Privacy notice
I have been given access to Sleep Care's Notice of Privacy Practices for Protected Health Information.

Payment is processed by Stripe. Sleep Care never sees your card number.
Choose either way. The order must include your full name, date of birth and the reference above.
Please do not send orders by regular email. We ship your test once we have both your payment and the order.
How it works

Ask your own qualified practitioner to order a home sleep test, using our order form or their own. An order is required.

Pay $260 by card through Stripe. The signed order reaches us one of two ways: you upload the order your practitioner signed, or your practitioner faxes it. Include your order reference either way.

Your Night Owl home sleep test is mailed to you. Wear it at home for up to 3 nights, then connect the Night Owl app and send your data within one week.
04
A board-certified sleep specialist interprets your results. Your diagnosis and our recommendation are sent to your primary care practitioner.

Patient responsibilities
